Support Coordination

At Chirpy Hearts we have qualified staff to have an understanding of your Support Coordination needs. Support Coordination is a capacity building support which helps you to:

  • Understand and use you NDIS plan to achieve your goals
  • Connect you with NDIS service providers, community, mainstream and other government services
  • Monitor service delivery, reporting requirements, fees and service agreements
  • Build your confidence and skills to use and coordinate your supports.
  • Coach participants to access and choose the right supports
  • Anticipate disability related barriers and help participants overcome those

A support coordinator or specialist support coordinator delivers support coordination services.

There are three levels of support coordination that can be included in your plan:

  1. Support connection
  2. Support coordination
  3. Specialist support coordination

Not everyone will have Support Coordination in their NDIS Plan. Some participants may have Psychosocial Recovery Coach instead (PRC). PRC’s we have at Chirpy Hearts, will either have lived experience of mental health or have long established experience in working with people with Mental Health diagnosis, achieving their recovery goals.
